Clinical Case Manager

Shelter Plus Care Clinical Case Manager (SPCCM) provides intake, eligibility and needs assessment for disabled, homeless and chronically homeless individuals and families seeking to enter the Shelter Plus Care Program. Provides structured goal-oriented casework for disabled, homeless and chronically homeless individuals and families who do not have a partnering supportive services agency. In addition, this position coordinates services with mental health, behavioral health, chemical dependency treatment and other social service programs and providers.

This position has a social justice component that requires critical thinking through the lens of racism and intersections with poverty. Knowing the core principles of antiracism and grounding those principles in everyday work are required job skills and core values.   • Provides consistent, intensive home-based casework for disabled, homeless and chronically homeless residing in permanent supportive housing, with the level of contact at the designated number of hours per week per client
  • Work in close collaboration with the Snohomish County Coordinated Entry System, CE Navigators and 211 to receive program referrals
  • After receiving program referrals, SPCCM will determine whether or not applicant has a supportive service agency. SPCCM will refer applicant to partnering agency
  • Eligible applicants without a partnering supportive services agency will participate in a comprehensive Needs Assessment. SPCCM will commence case management services
  • Assesses the physical, emotional, educational and emergent needs of clients with the goals of emotional and economic stability
  • Assists clients to determine, develop and carry out individual’s goals. Monitors goal progression on weekly basis
  • Assists clients to form short and long range financial plans and a realistic budget
  • Networks with other service providers to advocate for clients and establish suitable referral systems
  • Acts as liaison between client and other service providers on an as needed basis
  • Assists client in maintaining compliance with property lease and program
  • Participates in interdisciplinary staffings as scheduled
  • Arranges necessary letters or communications with other agencies or individuals to assist clients in securing income, emotional and housing stability
  • Maintains appropriate records for case reporting, client tracking and statistics
  • Participates in Snohomish County All Staff meetings, agency, department meetings and community meetings as needed or directed by supervisor
  • Provides structured exit interviews for clients to gather program feedback as well as help clients focus on long term goals. May support clients during the move in and move out inspections of apartment units.
  • Provides follow up case management for exited clients, including tracking of disposition
  • Maintains certification requirements including continuing education and re-certification applications
  • Attends trainings as directed to increase knowledge and skill. Attend required safety trainings and meetings; follow all safety and health guidelines, standards, practices, policies and procedures; and actively participate in the agency’s Emergency Preparedness program and activities 
  • Actively engages in agency-wide Race and Social Justice Initiative (RSJI), and strives for racially equitable outcomes; takes responsibility for creating and maintaining a safe and welcoming community by making room for people of color, trans and gender-non-conforming folks, and other populations who routinely encounter systemic oppressions
  •  Adhere to all Volunteer Services protocol relative to volunteer usage, recognition and monitoring

Qualifications & Core Competencies:
  • Bachelor’s Degree in human services, social work or related field and a minimum of 1 year of experience in social work, human services, advocacy and community building. MSW or Current WA State Agency Affiliated Counselor Registration preferred.
  • Experience working in Snohomish County, conducting group training and working with diverse populations
  • Comprehensive understanding of issues around homelessness, poverty and trauma, especially as they affect specific populations, including but not limited to: veterans, families, the elderly & disabled, people who live with chronic mental illness, youth, LGBTQI people, immigrants & refugees, and people of color.
  • Experience in providing crisis intervention services
  • Experience in providing comprehensive intakes and assessments
  • Knowledge of local human services, shelter, low income and affordable housing programs
  • Good oral and written communication skills and organizational and record keeping skills
  • Ability and willingness to work independently and as part of a team and to make sound judgement without on-site supervision
  • Commitment to working with and strengthening homeless families
  • Ability to maintain confidentiality

  • Experience working with communities of color and people from different cultures than your own

  • All positions at YWCA of Seattle | King | Snohomish are exposed to clients who have experienced or are experiencing trauma in various forms including but not limited to: domestic violence, sexual violence, homelessness, unemployment, financial hardship, etc.  As a result, staff are at risk of secondary trauma. Employees are encouraged to seek external support and maintain self-care when working indirectly or directly with clients
  • Continuously uses hands and wrists for fingering, handling, grasping and reaching in using telephones, computers, fax machines and other office equipment and supplies
  • Frequently uses speech, hearing, and sight abilities in exchanging information with clients, agency staff, employers, representatives of community organizations and other individuals in the community
  • Occasionally sits for extended periods while performing desktop activities                     
  • Occasionally stands and walks in performing duties in the office and in traveling to off-site meetings

Continuously over 80% of the time, Frequently 20-80% of the time, and Occasionally under 20% of the time.


Hours, Rate & Benefits:
• Hourly rate: Dependent of experience and qualifications
• Hours per week: 40 hours per week, full-time
• FLSA Classification: Non-Exempt.
• Excellent benefits package including medical insurance, retirement plan, plus generous vacation, holiday and sick leave plans
• At the time of hire, employees may choose to voluntarily enroll in the Fidelity 403b Plan. Typically after two years of employment, employees are eligible to participate in the YWCA Retirement Fund.
